Addison County Hospice Volunteer Services

Hospice Volunteer Services’ (HVS) primary commitment is to provide the support of trained hospice volunteers to people with terminal illnesses and their families. Hospice volunteers offer both practical and emotional support tailored to the individual needs of each patient and family. Most of these services are provided in the home setting, but we also provide care in nursing homes, community care homes, and the hospital. In addition, bereavement support services are offered to all residents of Addison County, whether or not a loved one received hospice care. We offer a variety of programs to assist people who are grieving, such as regular grief support groups, consultation, one-to-one volunteer outreach, special supportive and educational events, and the Phoenix Group (a monthly pot-luck gathering of widowed people for the purpose of social and community connection). HVS has a commitment to serve as a resource and leader in providing education and support throughout Addison County around issues of end-of-life care, death, and bereavement. We offer a variety of educational opportunities, national teleconferences, and publications, as well as being a resource to area schools, agencies and churches.

Addison County Transit Resources

Addison County Transit Resources (ACTR) has been the public transportation provider for the Addison County region since 1992. We serve all residents and visitors of the county through many diverse transportation services.

ACTR serves the general public via an extensive shuttle bus system, and has many programs that focus on specialized populations including elders, persons with disabilities and low-income families and individuals. In addition, ACTR partners with many area organizations and businesses to provide transportation for special public events.

Age Well

Age Well, formerly Champlain Valley Agency on Aging, is a non-profit organization that supports the efforts of people 60 and older in Addison, Chittenden, Franklin, and Grand Isle Counties. For over 40 years, we have provided Vermonters with the necessary support to manage their daily living needs, with the goal of keeping them active, healthy and independent. Committed to helping individuals age well, we reduce barriers by providing access to healthy meals, in-home care and community resources. Delivered by staff members and over 1,000 incredible volunteers, our sought-after services are designed to meet the diverse needs of our clients, their families and caregivers. We do not charge for services provided. As a nonprofit, we rely on donations and encourage clients to contribute if they are able to do so.

Community Health Services/Open Door Clinic

The Open Door Clinic of Addison, located in Middlebury, Vermont, provides a wide range of medical services for their patients. All patients must be previously qualified and visits to the clinic must be by appointment.

Elderly Services and Project Independence

Elderly Services is a non-profit agency dedicated to promoting high quality of life for elders, and to supporting elders in their efforts to live in their own homes despite frailty, chronic illness or disability. This includes:

  • Project Independence Adult Day Care Center
  • Eldercare Counseling
  • Eldercare Information & Consultation for Family Members
  • Geriatric Needs Evaluation
  • Geriatric Mental Health Counseling
  • Long Distance Care giving support — out-of-state resources
  • ESI College Lifelong Learning Program — interesting classes on a variety of subjects to keep your mind sharp.
  • Family Caregiver Support Groups — meets every 2nd Wednesday, at 7 p.m., and every 3rd Friday, at 12 noon. All are welcome!
  • Monthly Seminar and Lunch Club
  • Aging Education Programs

Project Independence provides daytime care for frail elders and respite and peace of mind for their families. Open 12 hours a day, Monday through Friday, 7 hours Saturdays, care includes nursing, transportation, delicious hot meals, therapeutic activities, entertainment and social work in a safe and caring environment.

Professional Nurses Service

Professional Nurses Service provides home health care to all ages statewide as well as supplemental staffing to Vermont schools, hospitals and businesses. “ProNurses” employs Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Licensed Nursing Assistants (LNAs), physical therapists (PTs) and unlicensed caregivers. Both home visits and continuous hours of care are available, 24/7, 365 days a year. Medicare and Medicaid certified, all major insurances are accepted as well as private pay clients. Vaccination and wellness services are also available to businesses and schools. Lastly, ProNurses offers education for individuals interested in becoming Vermont licensed as a nursing assistant.

RSVP (Retired Senior Volunteer Program) of Rutland and Addison Counties/Green Mountain Foster Grandparent Program

RSVP and The Volunteer Center is an “Invitation to Serve” program for people of all ages who want to meet community needs through meaningful use of their skills, talents, interests and knowledge in volunteer service to nonprofit organizations. RSVP/VC involves individuals in service that matches their personal interests and makes use of their varied life and professional experiences. RSVP/VC enables people to contribute to their communities and feel good about themselves through the rewarding experience of volunteering. Additionally, RSVP has implemented programs called “Signature Programs” to address pressing community needs like osteoporosis and child literacy issues.

Vergennes Residential Care

Vergennes Residential Care Home is a Level III care home on the city green in Vergennes. We offer an 18 bed facility with private and semi-private rooms, all within walking distance to local shops, churches and activities. Vergennes Residential Care Home provides home cooked meals, personal linen and laundry service, assistance with personal care, medication and transportation. The location is particularly well suited to participation in the many community activities offered on the green. An RN is available and on call.

Vermont 2-1-1

Vermont 2-1-1 is a confidential, statewide, health and human services information and referral service. Our mission is to promote the health and well-being of all people in Vermont, through a statewide information and referral system for streamlined access to community resources. Trained information and referral specialists offer personalized assistance by telephone; help solve problems; and provide information and referrals to government and local programs, community-based organizations, and support groups — 24 hours/7 days. Language translation services are available.

Just dial 2-1-1 (a local call from anywhere in Vermont) for personal assistance, information, and referrals to community resources throughout Vermont.
